INSURANCE

 

We do not submit insurance claims for our patients, but we do provide patients with the necessary form to submit to their Insurance company for reimbursement.  The coverage each insurance company provides is different for every individual; therefore, you may want to check  prior to the visit to determine the terms of your coverage.

MEDICARE

 

Mary Alice Volkert has chosen not to be a Medicare provider.  This means that medical nutrition therapy services offered by our office are not covered by Medicare.

APPOINTMENTS

 

The first visit is a two-hour consultation.  Follow-up visits are a one hour consultation. 

 

Payment is due at the time of service in the form of cash or check.  Mary Alice Volkert & Associates does not accept credit cards. 

 

There is no additional charge for a spouse or caretaker to accompany a patient for informational purposes during his or her appointment.  In fact, this is welcomed.

 

We ask that you give us 24-hours advance notice when re-scheduling or canceling an appointment.  We set aside this appointment time only for you and would appreciate the opportunity to schedule another person.
